All registration forms are on this page.  You can find more information about uniforms, extended care, hot lunch, school directory and more under the Parent Information.  To learn about our fundraisers, school and social events, please visit the Fundraisers and Events page. 

Below you will find our formal registration document. Please fill it out completely and return with your $175 registration fee (check made payable to Prince of Peace) and copies of birth and baptismal certificates. 

Medical forms and permission slips can be returned at a later date.

We are pleased to offer FACTS Payment Plans and Grant & Aid Financial Needs Assessment on the same platform.  After you create your FACTS, you will see a link to "start and application" on the home page of your account.
